At Horizon Harmony, we believe that everyone has the right to live with dignity, independence, and choice. Our Assist–Personal Activities service is designed to support NDIS participants with daily personal tasks, helping them build routines, develop independence, and live life on their own terms.

What is Assist–Personal Activities?

Assist–Personal Activities is a funded support category under the National Disability Insurance Scheme (NDIS). It is intended to provide help with personal daily tasks that a person may not be able to perform independently due to their disability.

This service can be provided in your own home, in shared accommodation, or out in the community. It includes both hands-on assistance and capacity-building support to help you gain skills over time. At Horizon Harmony, we tailor our support to match your unique lifestyle, preferences, and goals.

What We Can Help With

Our Assist–Personal Activities services include support with:

Our support workers are trained to provide high-quality care with professionalism, respect, and cultural sensitivity. Whether you need occasional support or daily assistance, we’re here to help you live a comfortable, supported, and empowered life.

Who Is This Support For?

This support is ideal for NDIS participants who need help with everyday tasks due to physical, intellectual, sensory, or psychosocial disabilities. If these activities are identified as part of your NDIS plan, we can work with you to ensure they are delivered safely and respectfully.

Assist–Personal Activities can be provided to individuals of all ages and can be delivered at home or in supported living environments, depending on your needs and goals.

Why Personal Support Matters

Daily routines are essential to health, wellbeing, and independence. But for many individuals with disabilities, these tasks can pose challenges. Our goal is not just to assist with personal care, but to build confidence and develop capacity over time.

With the right support, NDIS participants can enjoy more control over their lives, feel empowered in their routines, and reduce the reliance on family or carers.

The National Disability Insurance Scheme (NDIS) is an Australia-wide scheme that was designed to provide support to people with Disability. It provides funding to eligible people based on their individual needs. It is designed to help participants get the support they need so their skills and independence can improve over time. NDIS participant has an individual plan that lists their goals and the funding they have received. NDIS participants use their funding to purchase supports and services that will help them pursue their goals.
The National Disability Insurance Agency (NDIA) is responsible for assessing your eligibility to participate in the NDIS. To be eligible you must: Be under 65 years of age Be an Australian citizen or hold a Permanent or a Special Category visa Have an impairment or condition that is likely to be permanent (lifelong) and that stops you from doing everyday things by yourself

You will be asked how you want to manage your funds and supports in your NDIS planning meeting.
Your options for Plan Management are:

  • Manage your funding yourself - the NDIS will provide you with the funding to pay directly to the people and organisations that support you. This is called self-managed.
  • You can nominate a trusted person to manage your funding, called a β€˜plan nominee’
  • Use a registered Plan Management provider
  • Ask the NDIA to manage your funding for you, which is called agency managed.
